Scheduling and forms – for your convenience we have links to our most common forms as well as Patient Portal access. Please call our office with any questions.

 

REQUEST AN APPOINTMENT
Please call our office 501-588-1100 to schedule an appointment or use the email link to request a call back to schedule. Be sure to have your insurance information with you when you call or if you emailed us, when we call to schedule your appointment. We will verify your insurance before the first visit, and contact you for any problems, referrals required, deductibles and co-payments. Office scheduling has various hours and clinics depending on which Physician you see.

 

CANCEL APPOINTMENT
If you must cancel a scheduled appointment, please notify us at least 24 hours in advance to avoid a $25.00 missed appointment charge.

 

NEW PATIENTS
It is best to arrive 20 minutes before your scheduled appointment time. Present any completed paperwork to our receptionist. You will sign a consent, acknowledgement forms, and an authorization for release of personal health information as well as our Notice of Privacy Practices (HIPAA). If you would like a copy of the Notice of Privacy Practices, please ask for one.

Initial visit- please be sure to bring-Insurance Card(s), Drivers License or Photo Identification, completed Health History (this will be mailed to you prior to your appointment), All Current Medications you are taking. Bring the medications rather than just a list of them.

We will take a photo for identification to be placed in your chart.

 

TELEPHONE CALLS
We try to return calls as quickly as possible. Calls for our clinical staff are generally handled as the call is received, or a return call promptly will follow any message you leave, unless it is during patient clinic hours. Calls received during those times are returned as the schedule permits.

Calls received after 4 pm will be returned the following business day.

 

LAB TESTING AND TEST RESULTS
We encourage you to register on our Patient Portal-MyChart to receive lab and test results quickly. Please contact our receptionist for information on how to set up your patient portal account. Our nurse will only call you after instruction by your physician if your lab results are abnormal. Otherwise the results will be on your patient portal.

 

PERSCRIPTION REFILLS
Please contact your pharmacy and have them send an electronic refill request or fax the refill request to 501-588-1750. You should allow 7 days for prescription processing. If you require mail order refills, then you will need to turn those in to the nurse 14 days in advance for completion. For electronic prescribing we will need to have your current pharmacy information on file.

 

BILLING AND INSURANCE
Patient’s understand and agree they are responsible for anything their insurance does not cover such as, but not limited to deductibles, co-insurance, co-pay or any services not covered by the insurance plan. Most insurance plans are accepted at our clinic but some have restrictions. Your insurance coverage will be verified when you make each appointment. If there is a problem, we will contact you.

 

We cooperate fully with patients’ insurance carrier; however, it is important that you are aware of any restrictions specific to your plan. Please call your insurance carrier if you have questions about coverage including deductibles, co-payments or co-insurance amounts due by you at the time of service.

 

We do accept Visa, MasterCard, cash, personal check with valid ID or money orders. If you have questions regarding billing, please call 501-588-1100. They will direct your call to the appropriate department.
